Perbandingan Kadar Titer ASTO pada penderita kusta dengan ENL dan tanpa ENL

ABSTRAK

Latar belakang : ENL atau reaksi kusta tipe 2 merupakan komplkasi yang sering dijumpai pada kusta tipe BL dan LL. Salah satu pencetus timbulnya ENL adalah adanya infeksi bakteri dan telah diketahui bahwa  pada penderita kusta rentan untuk timbul infeksi streptokokus. Dilakukan tes ASTO untuk mendeteksi adanya antibodi serum terhadap antigen streptokokus.

Tujuan : Untuk mengetahui peran infeksi streptokokus sebagai pencetus ENL pada penderita kusta.

Metode : Dilakukan studi dengan disain potong lintang terhadap 32 penderita kusta dengan ENL dan 32 penderita kusta tanpa ENL yang dirawat di unit rawat jalan dan unit rawat inap RSU Dr. Soetomo Surabaya selama bulan April dan Mei 2008. Pada  seluruh penderita dilakukan tes ASTO, dan sebagai nilai normal dipakai patokan kurang atau sama dengan 200 IU. Pada kedua kelompok hasil tes ASTO dibandingkan dan dilakukan tes menggunakan Chi Square untuk melihat adanya perbedaan diantar dua kelompok tersebut.

Hasil : Hasil tes ASTO > 200 IU terdapat pada 53,1% penderita dengan ENL dan 18,8% penderita tanpa ENL. Terdapat perbedaan yang bermakna di antara kedua kelompok tersebut dengan p = 0,009. Perbedaan yang bermakna ini terutama terdapat pada kelompok BL dengan ENL dan tanpa ENL.

Kesimpulan : Terdapat perbedaan titer ASTO yang bermakna antara penderita kusta dengan ENL dan tanpa ENL.

ABSTRACT

Background : ENL or type 2 lepra reactions complicate lepromatous leprosy and borderline lepromatous leprosy. Bacterial infection is one of the factor known to precipitate ENL and patients with leprosy are probably more susceptible to streptococcal infection. ASTO test is performed to detect and measure serum antibodies against streptococcal antigen.

Objective : to study the role of streptococcal infection in leprosy patients with ENL

Methode : Using a cross sectional design study, 32 leprosy patients with ENL and 32 leprosy patients without ENL of Dermato-Venereology outpatient and inpatient clinic Dr. Soetomo Hospital Surabaya from April and May 2008 were examined for ASTO test. The ASTO test of leprosy patient with ENL and without ENL were compared and the Chi Square test were used to find out the differences between two groups.

Result : The ASTO titer > 200 IU was 53,1% in the group with ENL and 18,8% in the group without ENL. It show significant difference between two groups (p=0,009). The difference especially found in BL type.

Conclusion : ASTO titer is significantly different between the group with ENL and without ENL.
